**NEW** Employee Health Care Benefits Forms

   Effective January 1, 2016, all benefit eligible employees will have health care benefits with the Stark County Schools Council of Governments Consortium.
 
    All benefit eligible employees are required to submit the Application (Medical, Prescription, Dental, and Vision) and Enrollment Form (Life Insurance) by  Friday, November 6, 2015- Department of Human Resources / Attn: Lily Greco.
 
   Benefit eligible employees hired after July 1, 2015, are also required to submit the Spouse Eligibility Certification Form.

Professional Employment Application for the Youngstown City School District

All newly hired core academic teachers and instructional paraprofessionals should be highly qualified upon employment . Listed below are the links to access Youngstown City Schools' Application for Elementary, Secondary, Special Education, Administrator, Supervisor, and Special teachers.

The application must include the Highly Qualified Teaching Form (linked below to the ODE website) for your teaching area.

Ohio Department of Education - Licensure Application
 
Applications for Licensure can be found on the ODE Website, linked below. Please print, sign and have the application signed by Assistant Superintendent of Human Resources. (Adult, Pupil Activity Supervisor, 2-year license renewal, Educational Aide, Duplicate Request, Substitute License.)

Initial License must be signed by your college. Various applications - 5-year license, Supplemental, Alternative, etc.

Background Checks
 
All Licensure applications require a BCII/ FBI fingerprint check. Background checks are good for one year. Our Human Resources Dept. can complete this check.  
Please call for an appointment. (6926) (6927) $50 cash
Teacher must be in a Master's Program
- up to $500 per year Send the following to Human Resources Dept.:
Completed form, copy of fee paid and official transcript
